TDAP Shots for 6th Grade Students

The State of Florida requires that all students entering, repeating, or transferring into 7th grade have a Tetanus/Diphtheria/Pertussis (TDAP) booster shot. The TDAP booster must be
completed prior to entering the 7th grade.
Flagler Schools, in collaboration with the Flagler County Health Department, is
offering TDAP vaccinations at school. It’s free, easy, and offers our families the opportunity to complete this requirement and avoid missed time at school
and work.
TDAP vaccinations will be given at WES on Wednesday, April 20, and are voluntary. Each student will receive a notice to carry home indicating that s/he took the TDAP vaccination, if it was given.
The TDAP Consent Form must be returned to the school by Monday, March 28th. Students must have a signed TDAP permission form on file and be willing to sit for the TDAP
vaccination, without a parent present in order to participate. If your child’s school does not have the most current
shot record for your child, please provide it to the school registrar.
Please read the Vaccine Information Statement about TDAP for the most up-to-date information. If your child has received a
Tetanus/Diphtheria (TD) booster in the past three years, it is still recommended that they get
the TDAP vaccination at this time to give them additional protection from Pertussis.
